Remote position: November 2021 - current

• Independently manage incoming referrals across three Adult Support Programs in Darwin and complete all necessary intake assessments
• Efficient use of a client management system and referral
  register
• Autonomously complete all data and reporting to an exceptional standard for senior management
• Effective communication with key organisation stakeholders to ensure clients receive optimal support to address their mental health needs

Outreach position: May - November 2021

• Worked across three Adult Supports Programs simultaneously: Recovery Assistance Program, Psychosocial Support Program, The Way Back Support Service
• Provided one-on-one psychosocial support to Adults with
  mental health challenges or illness
• Provided intensive psychosocial aftercare support to people following a suicide attempt or in a suicide crisis for up to 3 months
• Implemented functional assessments to inform develop of an
  Individual Recovery Plan and Support Plan
• Liaised with community and clinical stakeholders to deliver wrap-around support to clients

• Communicated information clearly that reflected government advice/regulation
• Managed callers with heightened emotions

• Efficient time management skills and multitasking, especially during busy periods
• Able to work effectively in a team and adapt to changing situations
• Excellent communication with customers in person and on the phone
