Professional Practice 2: Westmead Hospital, General Medical
5 weeks (October 2023 – December 2023)
- Aseptic technique + wound care - competent in aseptic non-touch technique, basic wound care and associated documentation
- Medication administration - gained experience giving medication through different routes (including subcutaneous, oral, rectal, and eye drops) (excluding insulin and accountable drugs)
- Time management - utilisation of shift planners helped to manage my day with a patient load
- ISBAR handover - providing the afternoon staff with a handover of my patient load utilising the ISBAR format
Professional Practice 3: Blacktown Hospital, Surgical Ward
4 weeks (May 2024 - June 2024)
- IV medication administration - skilled in IV medication preparation and delivery via bolus, volume-controlled infusion, and IV container; confident in insulin administration, medication calculations, and sterile technique
- IV therapy management - competent in priming IV lines, and setting up the infusion pump
- 12-lead electrocardiograms - able to correctly place the appropriate ECG dots on a patient and attach the leads
- Pre- and post-operative care - able to care appropriately for a patient postoperatively, including the removal of sutures and staples
- Focused assessment - skilled in comprehensive focused assessments, including neuro, respiratory, cardiovascular, GI, and musculoskeletal systems
Professional Practice 4: St. Vincent's Private Hospital, Surgical Ward specialised in Urology, Gynaecology, and Breast Cancer
5 weeks (November 2024 – December 2024)
- Bladder and urological care - competent in bladder scanning, catheter care, removal of female catheters, and continuous bladder irrigation, with associated fluid balance charting
- Pain assessment & management - assessed and managed patient pain utilizing pharmacological interventions (S8 and S4D medications, PCA) and non-pharmacological interventions
- Drain management & removal - Monitoring and documenting drain output. Also provided complex wound care, including drain and suture removal
- Post-operative care and discharge planning - Provided post-operative care based on the surgeon's instructions, and also provided the patient with post-operative education on how to empty their catheter leg bag Completed discharge planning
- Pre-operative patient education - provided education to patients surrounding what care we are providing for them at the moment, when they are fasting, and what to expect after the surgery.
Professional Practice 5: Norwest Private Hospital, Intensive Care Unit (ICU)
4 weeks (May 2025 – June 2025)
- Respiratory and emergency support - supported respiratory interventions including high-flow nasal prongs, oxygen via nasal prongs, non-invasive CPAP, and observed airway management
- Care of an unconscious patient - assisted providing care to an intubated patient, which involved pressure area care, suctioning of the mouth, pain relief, and sedation if the patient was uncomfortable or restless
- Vascular access and fluid management - assisted with IV medication administration, TPN, fluid and electrolyte balance, CVP, and ECG interpretation
- Documentation and ICU flowchart - accurately documenting patient progress on the paper charts, and documenting patient vital signs, fluid balance monitoring, GCS score, and mechanical ventilation numbers on the ICU flowchart
- Assessment and deterioration - applying the A-E framework to complete an assessment on my patient to prioritize care, notify the ICU doctors of any unusual findings, and recognize and respond to any early signs of clinical deterioration
