Role Purpose:

  • To provide 24-hour specialist palliative care within the Brisbane area to patients with a life limiting condition, both within their homes and in Residential Aged Care facilities. Support of families and carers.

Key Achievements:

  • Case Manage patients within a specific area of Brisbane, providing expert palliative care
  • Share knowledge both within my team and externally.
  • Involvement in Multidisciplinary Care Meetings
  • Mentor, develop and manage staff to ensure quality care.
