• Provide trauma-informed support, behavioural guidance, and independent living skill development for high-risk young people in residential care.
• Collaborate closely with care teams, residential staff, and DFFH to implement client-centred plans and achieve positive outcomes.
• Support young people experiencing complex trauma, mental health challenges, disability, and substance use, ensuring safety and stability.
• Manage challenging behaviours using evidence-informed strategies and de-escalation techniques.
• Participate in multidisciplinary team meetings, contribute to case planning, and assist in risk assessment processes
• Maintain accurate records and reports in compliance with legislative and organisational requirements.
• Demonstrate resilience, initiative, and commitment to professional growth in a high-pressure, dynamic environment.

Temporary cover of House Coordinator responsibilities:

• Care and management support for young people in Conrad Home.
• Collaborative work with Care Team and other involved professional services including DFFH, PPU.
• Attending programme management meetings and providing reports/updates of Young People and Conrad home.
• Responsibility of coordinating and rostering carers in the home including supervision and daily support.
• Compliance with OH&S Policies and procedures.
• Reporting to management regarding safety concerns for young people.

• Support clients with daily living activities to enhance independence, quality of life and well-being within their homes, respite services and the community.

• Supported young people (14–22) in developing life skills, resilience, and independence through trauma-informed care.
• Delivered practical support including daily living guidance, problem-solving, and wellbeing-focused activities.
• Collaborated with families/whānau, social and education sectors, and community services to strengthen support networks.
• Maintained a safe, inclusive environment that promoted personal growth and positive outcomes for youth.
