I am a passionate, dedicated, and compassionate individual. I am interested in applying for Diabetes Clinical Nurse Consultant position at WMH. I am an energetic individual with strong nursing presentation skills, and excellent academic qualifications. I am a Credentialed Diabetes Nurse Educator, currently working at Werribee Mercy Hospital as causal staff member. I am looking forward to joining the wonderful, supportive team at WMH to continue my ongoing professional development.

• Providing inpatient and outpatient education, support and care to people living with diabetes and their families and/or carers
• Provide holistic patient centred care to people living with diabetes, including T1DM, T2DM, GDM and other types of diabetes
• Provide education and support to women with diabetes in pregnancy through individual or group education (New GDM or insulin start groups, GDM on call clinic)
• Provide phone support to people with diabetes, including insulin titration, self-management education and support through programs such as HITH and GDM on call
• Facilitate outpatient Diabetes Nurse Educator clinics such as Rapid Access DNE, Diamond Clinic Western Health Hospital
• Working as part of a multidisciplinary team that liaises with acute and community services
• Supporting patients with diabetes technology such as continuous/flash glucose monitoring
• Engages in ongoing professional development through in-service sessions
• Actively participates in developing and maintaining a positive learning environment
