I am keen to progress my career within my own business, Signature Occupational Therapy, whilst developing relationships with other service providers who have a strong emphasis on holistic patient care.
· Complete initial and complex assessments of people who present to hospital through the continuum of care;
· Complete hospital based functional capacity and cognitive assessments for the NDIS;
· Liaise with family, carers and significant others regarding previous function;
· Follow evidenced based practice and national guidelines for practice foundations;
· Patient flow and discharge planning;
· Education to patients, families, support agencies, treatment teams and other health professionals;
· Ability to work independently and within a multidisciplinary team;
· Independently manage a complex diverse caseload;
· Organisation, co-ordination and planning of individual and group therapy sessions;
· Organisation, facilitation and engagement in family and team meetings;
· Co-ordinate and actively participate in working party and committee meetings;
· Independently supervise level 1-2 Therapists and undergraduate students;
· Provide clinical supervision and mentoring support to new employees and allied health assistants, within the
· occupational therapy and multidisciplinary teams;
· Independently and within a team setting co-ordinate, engage in and complete quality improvement activities;
· Independently complete administration duties in a timely manner.
· Independently manage a large client caseload;
· Complete client assessments, including Functional assessments, Activity of Daily Living assessments, (including equipment prescription, service recommendation, task adaptation and home modifications), Pre-employment assessments, Worksite assessments, Ergonomic assessments;
· Liaise and co-ordinate client treatment and care with other health professionals (including Doctors, Specialists, Physiotherapists);
· Co-ordinate client return to work with health professionals, Employers, Insurers, Return to work Coordinators and the client;
· Communicate in an interdisciplinary team environment, including case conferences, and Work place meeting discussions and Case reviews;
· Individually evaluate clients' needs and compete individual service plans;
· Complete reports based on assessment findings, case conferences, and discussions with supervising manager.
· Independently manage a client caseload with various diagnoses and ages;
· Complete client assessments, including comprehensive assessments, sensory profiles and initial assessments;
· Write client case notes, assessment reports and letters to services;
· Complete both individual client and parent therapy sessions;
· Communicate in an interdisciplinary team environment, including case conferences, and family environment, including family meetings;
· Work with other health professionals through one on one and group interventions;
· Individually evaluate client's needs and compete individual service plans;
· Liaisons with other services including, schools, paediatrician's, general practitioners, community service providers;
· Assisting in the supervision and education of students.
