Challenging Behaviour & High Needs Client with Huntington's Disease

• Provide person-centred support to individuals with disabilities in line with their support plans, goals, and preferences
• Assist clients with daily living activities, including personal care, mobility, meal preparation, medication prompts, and household tasks
• Support participants to develop independence, life skills, and social inclusion within the community
• Act as a role model and mentor to junior support workers, providing guidance, on-the-job training, and support
• Coordinate shifts and ensure continuity of care across support teams
• Monitor client wellbeing and identify changes in physical, emotional, or behavioural needs
• Implement and review behaviour support plans and positive behaviour strategies
• Maintain accurate documentation, progress notes, and incident reports in accordance with organisational and NDIS requirements
• Liaise with families, allied health professionals, case managers, and service providers
• Ensure compliance with NDIS Practice Standards, organisational policies, and safeguarding requirements
• Respond to incidents, emergencies, and challenging behaviours using de-escalation techniques
• Promote health, safety, dignity, and rights of people with disabilities at all times
• Assist with service planning, reviews, and reporting as required

Spinal Injury Support
