I collaborate closely with allied health and NDIS professionals to ensure comprehensive care. I manage and guide support workers to deliver high-quality services tailored to client needs.

My focus is on helping clients achieve their NDIS goals while providing the necessary support for their daily activities and overall quality of life. This includes conducting frequent home visits, updating and creating Support Plans, performing multiple risk assessments, and actively participating in care team meetings, along with maintaining ongoing communication with allied health professionals.

Coordination Team Leader:

  • Serve as the first point of contact for all incoming referrals and inquiries from potential clients; complete client intake and conduct home visits
