Communicate and enrich the cooperation between pharmacists and patients.

Specific task include:

- Assist dispensing prescriptions

- Counselling on new medication

- Provide recommendation for selecting OTC medication (with pharmacist supervising)

- Handling medication stocks, stocktakes and checking expiration dates.

Assist with reviewing nurse request for medication supply

Assist with reviewing patient's medication on discharge.

Assist with taking medication history from patients, pharmacy and local GP.

Specific tasks include:

- Assist with medication supply for ward imprest.

- Compare patient's medication on discharge with current medication, past medication and doctor's plan using EMR.

- Communicate with patient, patient's carer or local GP to obtain medication history.
