I have 23 years of nursing experience in acute, chronic and remote nursing. My current position is managing the Respiratory Coordinated Care Program (RCCP) at Prince of Wales Hospital. Coordinating 4 staff and 400 patients to self-manage with chronic respiratory conditions. My achievements as a senior clinician and leader in change include successfully implementing innovative models of care and workplace systems to achieve record low patient hospital readmissions to Prince of Wales Hospital (POWH) Emergency Department. My success in the role contributed to achieving the leading patient flow rates in NSW. I am a results driven, team player with strong organisational and interpersonal skills. I have excellent time management and problem-solving skills and have the ability to handle multiple projects simultaneously with a high degree of accuracy.

  • Manage the Respiratory Co-ordinated Care Program (RCCP) including a multidisciplinary team coordinating 3 respiratory nurses and a physiotherapist and 400 patients with chronic respiratory conditions
  • RCCP data outcomes pre/post 2016 53% (n=58) of this patient cohort had fewer admissions in the 12 months after commencing RCCP, than the 12 months proceeding 32% reduction in the average LOS for admissions in the 12 months after RCCP commencing, than the 12 months preceding
  • This patient cohort used a total of 1,643 bed days in the 12 months prior to RCCP, compared with 1,537 bed days after RCCP (6% less)
  • Adaptation of the patient safety and isolation model of care from Emergency Department to Respiratory ward at Prince of Wales Hospital (POWH) (2016)
  • Developed an integrated referral system between Prince of Wales Hospital and chronic care outreach services across Sydney Local Health Districts
  • Submission for proposal of Community Facing Medical Assessment Unit for the Redevelopment Project Prince of Wales 2017
