I commenced working in the agency in 2016 as a Senior Planner and progressed to a Planner Supervisor role in 2018. Since then I have managed various teams and am currently supervising 11 staff members. My duties within this role consist of the following:
I undertook a position as a Team Leader within Disability Intake between 2010 and 2016. My duties within this role consisted of the following:
Compiling a monthly phone roster for intake staff.
Overseeing several staff members in the Intake team and supporting them in undertaking case management services for participants with disabilities.
Screening case management referrals
Providing a mentoring role within the Intake room to Intake staff members on a daily basis.
Supporting team members to manage escalations
I worked in Intake and Response as an Advanced Case Manager for approximately two and a half years.
Duties included:
- Short-term case management.
- Intake duties, including complex participant assessments
- Determining the participant’s eligibility for Disability Client Services and linking to relevant supports both external and internal.
- Referral and coordination of formal support services.
- Completing funding applications for participants experiencing financial hardship.
- Advocacy for participants
- Mentoring less experienced staff members and assisting with the training of new staff.
I worked as a hospital social worker based in the Aged Care rehabilitation ward at Bundoora Extended Care Centre (BECC) for a period of 4 months.
Duties included:
Conducting psychosocial assessments to determine the need for social work intervention, such as referrals for formal supports, and aged care packages.
- Coordination and chairing of Family Meetings, Care Plan Meetings, and Stroke Education Meetings.
- Discharge planning – involving referral to appropriate formal support services, or residential care, as required.
- Providing counselling to those patients who have significant social issues impacting their lives.
I worked as a Social Worker based in Aged Care on both Geriatric and Rehab wards at Broadmeadows Health Service for a period of 4 months.
Duties included:
- Psychosocial assessments of clients and determining the need for formal support or residential care
- Coordination and chairing of family meetings
- Working with families throughout the residential care
process
- Discharge planning, involving referral and coordination of formal support services, as well as referral to short and long-term Aged Care Packages as required.
- Providing counselling to those patients who are facing various social issues and working through issues that are currently having a direct impact on their lives.
- Completing aged care client records for patients requiring residential care as well as waitlisting patients for short and long-term aged care packages who require ongoing support in the community.
