• Supporting clients with activities of daily living (showering, dressing, personal grooming,

toileting)

• Meal preparation and assistance including peg feeding.
• Supporting clients to access the community and various recreational activities.
• Supporting clients with behaviors of concern and challenging behaviors as per their support plans.
• Medication administration and various documentation.
• Supporting clients with a focus on person centered active approach.
• Supporting clients with transfers and mobility with the use of various mobility aids.
• Assistance and support with general duties (laundry, vacuuming, cleaning)
• Supporting clients with disabilities to live comfortably, safely and independently in their own homes and in residential facilities.
• Supporting independent living and companionship
• Driving clients for various appointments and for recreational activities (community access)

• Support clients with personal care activities – including toileting, bathing, oral hygiene, dressing/undressing, continence management and other personal needs
• Support clients with dual diagnosis/forensic history .
• Support clients with high challenging behaviours such as verbal aggression, physical aggression, potential absconding and continence/toileting
• Ensure that the right type and the right amount of support is provided to allow the supported person to live with dignity, meet his own goals and follow his dreams.
• Support clients with domestic tasks – including cleaning, shopping, cooking and feeding
• Assist clients psychologically to bring positive attitudes and encourage good behaviour
• Engage with the supported person through conversation, day programs and activities
• Assist with mobility and use of ceiling hoist systems when require
• Implement behaviour intervention and support programs
• Assist clients in any physical and mental exercises and engaged them community based activities
• Monitor any changes in the client’s needs and reporting them
• Maintaining a safe living environment by minimising or removing safety risks and hazards
